I'd have to say the "fail" is letting inmates design the logo in the first place.

The article says they didn't design it, but they're contracted to print and apply them and that one probably got into the system and modified it.

Just checked my Spam folder in gmail making sure a message I'm expecting didn't get filtered. Found a reply to a thread I started on Google's support forum.
