I have several clients who had strokes who sort of described their early attempts to speak as you do here- their brains could form the thought clearly, but somewhere between thought and articulation there was a disconnect. Incredibly frustrating, but at the same time it made them appreciate the way things are supposed to work together in a whole new way. I like the baby/smile part especially.
