Obviously not the origin (I don't think) but the gesture gained some additional notoriety a few years ago when it was discovered that a high school yearbook committee had peppered that years edition with photos of people making the gesture. There was somehow outrage when it was explained to the adults in the area.

I had heard the first phrase you mention, but I otherwise feel so unenlightened!
