1. How many actual paper checks per month would you say you write on average? (Do NOT include online bill pay services)..

Actual paper checks at least 15 a month, sometimes more.

2. What is the split between household bill paying and still using them in stores at point of sale?.

Household billing, ie: phone, power, gym membership, all online.
Point of sale checks... maybe four a month.

3. Do you regularly use checks in a way that wouldn't seem to fall under either category in question 2? If so, please share if you're willing..

I write a check every week to the karate teacher,
once a month to the guitar teacher.
Every other week to the afterschool provider,
whenever I need to for school lunch, and whatever I have to pay for at school. (bookfair, pta dues, fundraisers)
At work for other fundraisers, Avon.

4. How many accounts do you write checks against?.
