I was thinking Misery, but I liked the book a tad bit better. In the book...

if I remember this right, she cuts his foot off not just breaks it. I found that to be more of an impact. Also, she dies trying to grab a chainsaw which is even more horrid in my opinion.

But I do love both the book and the film.
