While it is funny that Bush is showing the shocker, it is even funnier if (as I just read elsewhere) that is the official hand gesture of Arizona State University (it is supposed to be the pitchfork of their mascot sun devil).

I have learned many things from that photo and the resulting research about it. It sure doesn't look like a pitchfork though.

I would like to research the origins, but I am not about do so from work.

If you haven't encountered it yet, add to your searches the phrase "two in the pink, one in the stink" or "two for you, one for poo."

Obviously not the origin (I don't think) but the gesture gained some additional notoriety a few years ago when it was discovered that a high school yearbook committee had peppered that years edition with photos of people making the gesture. There was somehow outrage when it was explained to the adults in the area.
