You're getting between $30 and $60 back depending on marital and kid status if you have had a phone line between 2/28/03 and 7/31/06 you're [most likely*] getting a refund.

*it doesn't specify if you needed to have made long distance calls in that time.

If you happen to make a lot of long distance calls, and have access to your phone records over that period, it might be worth your while to fill out form 8913 and claim your actual ammount rather than the $30-$60 standard refund.
