Nope, just the disk.

It says to mail it with out the case, so you're in luck. But it's only for a selection of over 100 WB movies titles (not ANY DVD). And free shipping if you spend over $30, so send in at least 7 at once!

Just to clarify, you can SEND any non-kevy dvd, you can only GET movies from WB that are on the list.

Thank you, I did have that wrong. That's very cool, I have some DVDs I don't want and wouldn't even sell on eBay. Not to mention that you can go to Big Lots and buy cheep DVDs, send them in with your $4.95, and still come out ahead!

Oh, and it's $35 so you'll want to send in 8 DVDs, as long as this deal qualifies toward the purchase amount that is. I'll have to take a look at their list to see if I want any of these movies! Perhaps some Christmas gifts!
