I think I'd cringe, too.

The bloody nipples come from not protecting them and have your shirt chafe them for 26.2 miles until they bleed.

Putting a large bandage over them BEFORE the race is the best thing to prevent I believe.

I'd think having them surgically removed would be more of a guarantee.
