I'm confused about who can see who. Is it whoever is in your circles can see everyone else (like before FB made friend lists public) or is it anyone can see everyone?

By default, anyone can see everyone.
There is a setting that allows you to keep strangers from seeing your friends
Once someone is your friend you they can see all your friends
No one can see what circles you have placed them in.
